Registration Fees

(Fees worksheets available below, on-line registration will be available May 13, 2009.)

County Extension Offices must complete the Registration Fees Worksheet and Residence Halls Fees Worksheet and return to PASE/Life Challenge at the address on the fees worksheets. Registration Fees worksheets and accompanying checks must be postmarked by June 15, 2009.

INSTRUCTIONS FOR COUNTY OFFICES FOR SUBMITTING REGISTRATION FEES

I. Residence Halls

A. Residence Hall Fees worksheet must be completed indicating the total number of male youth, female youth, adult males and adult females for each night.

B. Health and Permission forms, which include all necessary medical information and code of conduct expectations must be completed for each youth staying in the dorms. Forms MUST BE SIGNED by parents/guardians and the participants!

C. Send the fees worksheet, health and permission forms, and check to PASE/Life Challenge.

II. County teams:

A. Team fees: When all teams have been entered, complete the registrations fees for the teams and total on the contest registration total line.

B. Meals: Complete the number of people eating each meal. 

With the support of the Nebraska Investment Finance Authority, PASE/Life Challenge is able to provide lunches at the cost of $1 to contestants.  Staff and volunteer helpers will be offered a complimentary meal. The Monday evening picnic will be free of charge for all contestants, volunteers and special guests.  We will need to have an accurate meal count. Meal counts will be based on numbers reported on the fees worksheet.

C. Send check and worksheet to PASE/Life Challenge. Registration fees must be postmarked by June 15.

III. Deadline:

A. Fees worksheets and accompanying check must be postmarked no later than June 15.

B. Health and permission forms (medical release) must be completed before participants can be checked into residence hall. These should be sent with registration fees.
