• This form must be completed and SENT IN ADVANCE for all participants staying in the residence hall, except chaperones (over age 18).

• It is recommended that this form be completed for ALL participants in PASE and Life Challenge.

• County staff or chaperones may retain the forms for those youth not staying in the residence halls.
